RCFA Root Cause Failure Analysis digs deep to uncover the real reason behind system failures, not just the symptoms. It’s a structured, evidence-based approach to understanding why equipment, systems, or processes fail, and how to prevent them from happening again.
As Eberhard Lucke of Lucke Consulting Technology Services (LCTS) explains, RCFA is essential for eliminating chronic issues, reducing unplanned downtime, and improving long-term asset performance. Without it, you risk addressing surface-level problems while the root cause continues to trigger costly disruptions.
RCFA goes beyond quick fixes and temporary patches. It involves:
Data Collection – Gathering failure records, inspection data, operating logs, and witness input
Timeline Reconstruction – Understanding the sequence of events leading up to the failure
Cause-and-Effect Mapping – Using tools like the “5 Whys,” fault trees, and fishbone diagrams
Verification – Validating hypotheses through testing, inspection, or simulation
Corrective Action Planning – Recommending systemic improvements based on verified root causes
These steps help eliminate guesswork and ensure that the solution targets the actual failure mechanism, not just the most visible symptom.
RCFA is especially valuable when:
The same failure keeps happening despite repairs
Unexpected shutdowns are affecting production targets
Equipment performance is declining without a clear reason
There’s a safety or environmental risk tied to recurring issues
At LCTS, we support clients with RCFA services that are both technical and strategic. Our engineers collaborate with maintenance teams, operations staff, and safety managers to investigate failures from all angles—mechanical, process, operational, and human factors.
We help clients:
Reduce downtime and maintenance costs
Avoid repeat failures
Extend equipment life
Improve plant reliability and safety
Create a culture of continuous improvement
“RCFA isn’t just about fixing what’s broken; it’s about learning from it. It’s how high-performing plants stay that way.” – Eberhard Lucke
RCFA fits seamlessly into the plant lifecycle as a tool for both operational excellence and risk reduction. It empowers teams to think beyond symptoms and address the real drivers of performance loss.

The personnel that attend the RCA are crucial since all ideas or inputs were important to have a good quality RCA…But my question is…..if somebody from the group has more vocal and the his or her idea will be prevail….and evidence also is more important to justify the possible cause…and the weak side is pointing to other department for them to scape for there failure to performed their job..
